*ALL medications must be delivered by an ADULT ONLY to the SCHOOL NURSE or HEALTH AIDE ONLY. *  

Students must never possess any type of medication without documentation from a physician allowing him/her to do so. Only school health personnel are permitted to receive medication. The Blake Elementary School nurse’s hours are usually Monday through Friday (school days only) from 8:45 AM-3:30 PM. PH #330-636-3904. Please remember only the nurse can accept medication between 8:45 AM-3:00 PM.

If it is necessary that you drop off medicine at times other than these, the nurse will not be able to process it until she arrives at 8:45 AM.
