    New Student Registration

    2023-2024 (Current) School Year ONLY

     

    Registration for 2024-2025 (Next) School Year will Open on February 1, 2024.

     

    Medina City Schools utilizes an online student registration system called FinalForms.   You can access the appropriate registration link below to register your student  If you have any questions, please contact Registration at 330.636.3100.

     
    To register a student for the 2023-24 school year, please follow the instructions below. 
     
    Please Note:  Only the custodial parent/legal guardian can officially enroll a student. (Step-Parents are not able to register students.)  
     
    Before you get started, please ensure that your address falls within the Medina City Schools boundaries, as we cannot enroll students outside of the District. 
     

    REGISTRATION FOR GRADES K-12

    To register, click here.  NOTE:  Please be sure to click the blue "New Account" box when registering your student and choose the correct school year.

     

    EXISTING FINALFORMS PARENT 

    If you already have a FinalForms account with the school district (for example, an athletic account or older sibling), please use that existing account to log in and +Add Student.  To register, please click here.

     

    PRESCHOOL REGISTRATION

    Residency Affidavit - IF YOU ARE UNABLE TO SUBMIT AN APPROVED PROOF OF RESIDENCY DOCUMENT, YOU ARE REQUIRED TO COMPLETE THIS FORM. FORM MUST BE NOTARIZED. (Approved proof of residency documents: all pages of a current utility bill, mortgage contract, purchase contract, all pages of a lease agreement with signatures, insurance statement on the dwelling, or a letter from your builder indicating a possession date within 90 days of enrollment in the district.)
