    2024-2025 School Year 


    Medina City Schools utilizes an online student registration system called FinalForms.   You can access the appropriate registration link below to register your student.  If you have any questions, please contact Registration at 330.636.3100 or

    To register a student, please follow the instructions below:  
    Please Note:  Only the Custodial Parent / Legal Guardian can officially enroll a student. (Step-Parents are not able to register students.)  
    Before you get started, please ensure that your address falls within the Medina City Schools boundaries, as we cannot enroll students outside of the District. 
    IMPORTANT:  Once you have completed and signed all of the forms in FinalForms and have uploaded your documents, we will be reviewing the registrations in the order they are received.  You will receive an email if anything is missing or incomplete.  You will also receive an enrollment email when your student is officially registered into our student database.  We use email to communicate with you about your student's registration, so please be sure to monitor your email for updates.

    NOTE:  Please be sure to click the blue 'New Account' box when registering your student and choose the correct school year.



    If you already have a FinalForms account with the school district (for example, an athletic account or older sibling), please use that existing account to log in and click '+Add Student.'  Please do not re-create a student in FinalForms -- Call 330.636.3100 or email for help.

    Residency Affidavit - IF YOU ARE UNABLE TO SUBMIT AN APPROVED PROOF OF RESIDENCY DOCUMENT, YOU ARE REQUIRED TO COMPLETE THIS FORM WHICH MUST BE NOTARIZED. (Approved proof of residency documents: all pages of a current utility bill, mortgage contract, purchase contract, all pages of a lease agreement with signatures, insurance statement on the dwelling, or a letter from your builder indicating a possession date within 90 days of enrollment in the district.)
